Zepbound and Wegovy: what each costs, and what your plan is likely to do
Wegovy is the cheaper cash price at every published tier. Novo Nordisk sells Wegovy through NovoCare at $199 a month for the two starting doses if you are new to it, stepping up to $349 a month after the first two fills from 31 December 2026, and the Wegovy pill is $149 a month at 1.5 mg.
What that is based on
Eli Lilly sells Zepbound through LillyDirect at $299 for the 2.5 mg starting dose, which Lilly states is not approved as a maintenance dose, and $399 to $699 at the doses people stay on. Lilly’s purchase offer brings 7.5 mg and above to $449 a month, and it holds only while every refill lands within 45 days of the last one. Coverage is closer than the price. Across the 51 US Medicaid jurisdictions this site has read, semaglutide is covered subject to prior authorization in 13 and refused in 12; tirzepatide is covered subject to prior authorization in 11 and refused in 11. Both are approved for chronic weight management, and each also carries an indication the other does not: Wegovy for cardiovascular risk reduction, Zepbound for obstructive sleep apnea.

Wegovy is cheaper at every published tier, and the gap narrows once Lilly’s purchase offer applies: $449 against $349. What neither figure tells you is which one a prescriber will put you on, or which your plan will fund, and this page does not answer either.
