What Zepbound costs without insurance in 2026
Lilly sells Zepbound directly for $299 to $699 a month depending on the dose, and the number you will have seen quoted, $299, is the starting dose. Lilly states on its own page that 2.5 mg is not approved as a maintenance dose, so it is not the price you pay once you settle. At the doses people stay on it is $499 to $699. There is a $449 offer on 7.5 mg and above, and it holds only if you reorder within 45 days of your last delivery, every time.

Lilly self-pay for a one-month supply of the single-patient-use KwikPen, re-read from Lilly’s own page on 26 August 2026. 🔴 Lilly now advertises this as “$299 to $449 per month”, and the $499 and $699 prices survive only in a footnote as what applies if a refill lands more than 45 days after the last one. So the headline range is the discounted one and it depends on never being late. Taxes and fees may be added, and the KwikPen requires a separate pen needle purchase. The $299 figure covers the KwikPen or vial, not the single-dose pen. The savings card, which is a separate thing, expires 31 December 2026 and is capped at 11 fills a year.
