Does UnitedHealthcare cover GLP-1s for weight loss?

UnitedHealthcare calls its weight-loss program "an optional program that is put in place for clients or businesses that have elected to cover weight loss products". If your employer did not elect it, there is nothing to prior-authorize.

Your employer decides, not UnitedHealthcare. On most commercial plans, whether weight-loss medication is covered at all is chosen by whoever buys the plan. The criteria below apply only once that choice has been made, so the first document to read is your own plan's benefit summary.

What it takes to keep the coverage

You have to show it worked. UnitedHealthcare renews Wegovy and Zepbound only on documented weight loss of 5% or more of your starting weight, plus continued lifestyle changes. The first approval is 5 months for Wegovy and 6 for Zepbound, so that is the window. Saxenda needs 4% and Qsymia 3%. Renewal then runs 12 months.

If weight loss is excluded

If your plan excludes weight-loss drugs, three doors stay open: Wegovy for cardiovascular risk reduction where you have established cardiovascular disease, Wegovy injection for MASH with fibrosis at stage F2 or F3, and Zepbound for moderate to severe obstructive sleep apnea. UnitedHealthcare keeps separate criteria for exactly this.

States that require coverage anyway

These states mandate coverage regardless of what the plan sponsor elected: California, New Mexico, North Dakota, New York.

Worth knowing

North Dakota fully insured Essential Health Benefit plans are much stricter than the standard program: they require a BMI of 40 or over.

Asking about one medicine instead

If the answer is no. Around 45% of initial prior authorizations for these drugs are refused, and 35 to 45% of first-level appeals are overturned. A refusal is not the end of the process, and the cash-pay routes are priced on this site alongside it.
