What insurance covers Zepbound

Whether your insurance covers Zepbound is usually decided by your employer, not by the insurance company whose name is on your card. UnitedHealthcare calls its own weight-loss program "an optional program that is put in place for clients or businesses that have elected to cover weight loss products".

What that is based on

Aetna states that many of its plan descriptions "specifically exclude services and supplies for or related to treatment of obesity", and that under those plans claims for weight reduction medications "will be denied based on that exclusion". So the first document to read is your own plan’s benefit summary, not the insurer’s clinical policy. Where a plan does cover Zepbound, it almost always covers it only after a prior authorization, and most plans then ask you to have lost a set percentage of your starting weight before they will keep paying.

Questions people ask

How do I find out if my plan covers Zepbound?

Read your own plan’s benefit summary rather than the insurer’s clinical policy, and look for whether weight-loss medication is excluded as a category. On most plans bought through an employer that exclusion is chosen by the employer, so two people with the same insurance card can get different answers. The insurer’s published criteria only tell you what happens once weight-loss medication is covered at all.

What does a Zepbound prior authorization usually ask for?

Across the payers audited on this site, a Zepbound prior authorization typically asks for a BMI of 30 or over, or 27 or over with a weight-related condition, together with a record of lifestyle measures alongside the medication. Individual payers add their own tests, and Aetna sets the hardest one found here: taking part in a weight management program with follow-up for at least six months before starting the drug.

Why would insurance stop covering Zepbound after a few months?

Most plans reauthorize rather than renew automatically, and the common test is that you have lost at least 5% of your starting weight by the first review. UnitedHealthcare’s own program sets an initial authorization of six months for Zepbound and then requires that measured loss to continue. This is the part almost nobody is told before they start.

Does Medicare cover Zepbound for weight loss?

Medicare Part D has never paid for a drug prescribed only for weight loss. The Medicare GLP-1 Bridge is the program that changed what is available, it runs to 31 December 2027, and this site tracks it on its own page. Zepbound is also approved for moderate to severe obstructive sleep apnea, which is a different indication with a different coverage answer.

What if my insurance refuses Zepbound?

Around 45% of initial prior authorizations for these drugs are refused, and 35 to 45% of first-level appeals are overturned, so a refusal is a stage in the process rather than the end of it. The cash-pay routes are priced on this site alongside the coverage answer, and the cheapest of them is rarely the one being advertised.
