Does Aetna cover GLP-1s for weight loss?
Aetna states it plainly: "Many Aetna plan benefit descriptions specifically exclude services and supplies for or related to treatment of obesity or for diet and weight control. Under these plans, claims for weight reduction medications... will be denied based on that exclusion." So the first thing to read is your own plan document, not Aetna’s criteria.

What it takes to keep the coverage
Continuation asks that you have titrated to a stable maintenance dose and that your BMI has fallen from baseline, or that you have kept the reduction you already made. Aetna cites the Endocrine Society guideline behind it, which is the source of the 5% test the other payers use: below 5% at three months, the guideline recommends stopping.
Worth knowing
Aetna is owned by CVS Health, so the CVS Caremark formulary above often sits behind an Aetna plan. They are two different decisions: the formulary says which drug is preferred, the benefit design says whether weight-loss drugs are covered at all.
