Does Blue Cross Blue Shield cover GLP-1s for weight loss?

Blue Cross Blue Shield is not one insurer, so there is no single answer. The Blue Cross and Blue Shield Association describes itself as "a national federation of independent, community-based and locally operated Blue Cross and Blue Shield companies", and each of those companies sets its own coverage.

Who decides, in their own words

Some have stopped covering Wegovy and Zepbound for weight loss altogether: Blue Cross and Blue Shield of Vermont made them a benefit exclusion from January 1, 2026. Others leave the decision to each employer: Blue Cross and Blue Shield of Texas tells employer groups that for weight management "plan coverage is optional".

If weight loss is excluded

A Blue plan that excludes weight-loss medication has not always excluded the drug. Blue Cross and Blue Shield of Vermont keeps GLP-1s covered with prior approval for type 2 diabetes, and for adults with diagnosed cardiovascular disease and obesity whose provider prescribes Wegovy to reduce the risk of certain cardiovascular events.

Worth knowing

Blue Cross and Blue Shield of Texas answers its own members’ question "Does insurance cover GLP-1s for weight loss?" with "Most plans don’t cover GLP-1s for weight loss", and says that where a plan does, "you likely will need approval from your health plan before you start treatment". Federal employees are covered by a separate Blue plan, the Federal Employee Program, which this site reads on its own page.
