Form Health: what it costs and how it works

Form Health was one of five independent telehealth companies Eli Lilly listed on its own site for obesity care. It is not on that list any more: Lilly’s panel showed four options on 26 August 2026 and five on 30 August 2026, and Form Health is not on either.

The pricing and the detail

The fifth place went to Counsel Health, which is a different company. Its own site publishes a self-pay plan at $299 a month, which buys clinician visits, a registered dietitian, unlimited messaging and the platform. Lab work and medication are charged on top. It says visits, labs and medication are covered by most major private insurance and Medicare and that you pay any copay or deductible, and it publishes no figure for what that copay is.
