Form Health: what it costs and how it works

Form Health was one of five independent telehealth companies Eli Lilly listed on its own site for obesity care. It is not on that list any more: Lilly’s panel shows four options on 26 August 2026 and Form Health is not one of them. While it was listed, Lilly published its price as $299 a month for self-pay, or a typical copay of $10 to $100 a month where commercial or Medicare coverage applied. That was the care and not the medication. Nothing here is a current price, and Form Health has not been re-priced from its own site.
