What Ozempic costs without insurance in 2026
Novo Nordisk lists Ozempic at $1,027.51 a package, the same figure against every dose and both forms, and almost nobody pays it. Through NovoCare Pharmacy the published self-pay prices are $149 a month at 1.5 mg, and $199 a month for the 0.25 mg and 0.5 mg pens if you are new to the offer.
What that is based on
That $199 covers the first two months. From 31 December 2026 it becomes $349 a month for the 0.25, 0.5 and 1 mg pens and $499 a month for the 2 mg pen. Novo publishes no self-pay price at all for the 4 mg and 9 mg doses, which is worth knowing before you plan around one. Those dates are printed in Novo Nordisk’s own footnotes, so a price quoted anywhere without one may already have moved: the equivalent Wegovy offer ended on schedule in August 2026.

NovoCare Pharmacy self-pay, read from Novo Nordisk’s own pages on 12 September 2026. List price is $1,027.51 a package against every dose line, injection and tablet alike, and it sits on a separate page reached from a "List price explained" link rather than beside the price you are offered. The $199 covers two fills and then steps up on 31 December 2026. With commercial insurance a separate offer pays as little as $25 a month for up to three months, capped at $100 of savings, and government beneficiaries are excluded. A month is not one unit on this page: Novo defines it three different ways, as 1 bottle of 30 tablets, as 1 box of 1 pen, and as both together.

Across the 51 US Medicaid jurisdictions this site has read, semaglutide is covered for weight management subject to prior authorization in 13, refused in 12, and covered for a diabetes indication only in 22
Where you can actually get it
Three ways to pay cash for the same medicine. The cheapest price on the drug is not the cheapest way to get it, because two of these do not include a prescriber.
