Nebraska: what Medicaid covers

Nebraska has no weight-management drug class at all. Every GLP-1 sits in the diabetes class, and the criterion for the whole class is a diagnosis of type 2 diabetes. That is a different mechanism from the states that published an exclusion, and it lands in the same place for somebody without diabetes.

Semaglutide Other indications only Ozempic preferred, Rybelsus non-preferred. The class requires a diagnosis of type 2 diabetes and a failed trial of metformin, or established cardiovascular disease with diabetes
Tirzepatide Other indications only Mounjaro non-preferred, behind a failed trial of two preferred agents and an HbA1c of 7 or over. Zepbound is not on the list
Liraglutide Other indications only Victoza preferred and generic liraglutide non-preferred, both under the diabetes criteria. Saxenda is not on the list
If the answer is no. Around 45% of initial prior authorizations for these drugs are refused, and 35 to 45% of first-level appeals are overturned. A refusal is not the end of the process, and the cash-pay routes are priced on this site alongside it.
