Medicaid coverage
Missouri: what Medicaid covers
Missouri’s MO HealthNet began covering select GLP-1 weight-loss drugs for members aged 12 and older. Zepbound is the preferred agent and Wegovy is covered as a non-preferred drug with prior authorization, both requiring a BMI of 30 or more, or a BMI of 27 or more with a weight-related condition, or MASH. Saxenda is not on the list, and the state’s own policy says anti-obesity drugs not listed remain excluded.
| Drug | Position | Criteria |
|---|---|---|
| Semaglutide | Prior authorization | Wegovy is the non-preferred agent on the obesity GLP-1 PDL. Prior authorization for members 12 and over with a BMI of 30 or more, or a BMI of 27 or more with a weight-related comorbidity, or MASH with F2/F3 fibrosis Changed April 23, 2026 |
| Tirzepatide | Prior authorization | Zepbound is the preferred agent on the obesity GLP-1 PDL. Prior authorization for members 12 and over meeting the BMI or MASH criteria Changed April 23, 2026 |
