Arizona’s AHCCCS excludes medications used for weight loss treatment from the pharmacy benefit outright, and its prior authorization criteria say a GLP-1 requested for weight loss is denied as a plan exclusion. Wegovy can be approved with prior authorization to reduce cardiovascular risk, but none of the three brands is covered for weight loss.
