Maryland answers this with the form itself. There are three boxes on it: cardiovascular disease and MASH for Wegovy, sleep apnea for Zepbound. There is no box for weight management, and a prescriber cannot write one in. The form still asks them to attest to a failed weight-loss attempt in the past six months.
