Utah: what Medicaid covers
Utah is the one state on this grid with a coverage cliff already behind it. Its prior authorization form covers Wegovy for cardiovascular risk and for MASH, and Zepbound for obstructive sleep apnea, and those routes are unaffected. Weight loss was different: it was a legislative pilot, it was open only to people who already held a weight-loss authorization granted between 1 July 2025 and 30 June 2026, and the form makes the prescriber attest they know coverage may not continue past 30 June 2026. That date has passed. The form is still the one Utah serves and it was last updated on 1 June 2026, but the state has published nothing saying what happened on 1 July, so the weight-loss answer here is genuinely unresolved rather than negative.
