Iowa: what Medicaid covers

Iowa’s July 1, 2026 preferred drug list names Wegovy and Zepbound as preferred products, but its comment legend says the weight-loss indication is not covered and its prior authorization criteria say payment for excluded medical uses such as weight loss will be denied. Wegovy can be approved for cardiovascular risk and MASH, and Zepbound for obstructive sleep apnea. Saxenda is not named in either document.

If the answer is no. Around 45% of initial prior authorizations for these drugs are refused, and 35 to 45% of first-level appeals are overturned. A refusal is not the end of the process, and the cash-pay routes are priced on this site alongside it.
